Form KS-11 is a unified form of act, which indicates that any construction project is completely completed and ready for operation. It is filled out only after the construction customer has carried out the appropriate check and documented the results of all inspections, tests and measurements performed. It is this information that forms the basis of the act in the KS-11 form.

Filling out the KS-11 header
The first part of the KS-11 act includes information about the organization performing the construction project. Before entering its data, you should number the act and put the date of its preparation in the required cells (in two places). Next, enter the full name of the contractor company, as well as its OKPO code (from registration documents). Below are the positions and names of those responsible for acceptance of the constructed facility. officials on the part of the contractor and the customer.
We fill out the conditions for signing the KS-11 act
The second part of this document contains the conditions for signing the act, namely:
Paragraph 1. Here enter the full name of the construction project on which the contractor worked, indicating its detailed location (city, street, house/building).
Point 2. This includes the name of the government agency that issued the building permit.
Point 3. This line contains information about subcontractors, that is, those companies that also took part in this construction under subcontract agreements.
Point 4. Here you must indicate the full name of the company - the general designer who created the design and estimate documentation for the construction of this completed facility, as well as, if any, other organizations that took part in the development of this documentation. If there are no other organizations, you need to make a note that all design and estimate documentation was created entirely by this particular company.
Point 6. This line includes information about the company that approved the design and estimate organization (usually the construction customer, but sometimes an investor or other interested entity), as well as the approval date and document number.
Point 7. Here you need to enter the timing of construction and installation work, indicating specific dates the beginning and end of construction.

IN separate section we can highlight the eighth paragraph of the acceptance certificate. It includes two tables. The first of them contains six columns and concerns any construction projects, with the exception of residential buildings.
Column 1. Here you need to enter the name of the main indicator of the construction project: power, productivity, capacity, production area, volume, throughput, number of jobs, length, etc.
Column 2. Here you need to specify the unit of measurement of the previous indicator ( square meters, cubic meters, etc.)

Point 12-13. Here the numbers indicate the final cost of the construction project according to the design and estimate documentation (broken down by the cost of construction and installation work performed and the cost of equipment, tools and inventory).
Point 15. Any additional conditions, accompanying the fact of acceptance of the constructed building/structure, if any.
After all points are completed, the act must be endorsed. What is the KS-11 form?
The acceptance certificate for a completed construction facility is used to reflect in the accounting of the facility in respect of which all the necessary construction and installation works have been completed or reconstruction has been carried out. The form is unified by the State Statistics Committee and has approved form OKUD 0322003.
Important! This document must be drawn up by the customer after the condition of the object has been checked and the results of all measurements have been taken into account, since it is the test results that represent the main information reflected in the report.
The header of the document reveals the following information:
- Number and date of drawing up the act;
- Name of the organization;
- OKDP code;
- Other codes (company, site, facility).
- Name of the customer and contractor carrying out construction and installation work;
- Name and address of the construction site;
- A document confirming permission to carry out construction;
- Information about subcontracting companies that took part in construction and installation works;
- The company that compiled the design and estimate documentation;
- The organization that provided the initial information for the design and estimate documentation;
- The body that approved the design and estimate documentation for the construction project;
- Timing of construction and installation work (start and completion).
- The next step in filling out the document is entering information into the tables. They are presented in the amount of two pieces. You must fill out the one that corresponds to the category of the constructed object (residential or non-residential).
For non-residential properties, the “Option A” table is used. In this case, you will need to reflect the following data:
- Name of the construction and installation work object;
- The main indicators characterizing the construction and their units of measurement;

The table intended for information about residential premises (Option B) has a similar structure. However, the names of the indicators that require comparison are already included in the table. - Number of the application disclosing information about the equipment installed at the facility;
- Number of the application informing about the supplied communications (sewerage, cold and hot water supply, electricity supply, etc.);
- The table shall reflect data on the measures taken to develop infrastructure and plant greenery at the site (type of work, volume and deadlines);
- Next, information about the cost of the construction or reconstruction project must be disclosed;
- Information on the value of fixed assets;
- Additional conditions (indicated if available).
At the bottom of the document, it is necessary to indicate the positions and full names of the persons responsible for the delivery and acceptance of the construction project, as well as the signatures and seals of the construction and installation contractor and the customer.

Question No. 1 Is the company required to use the unified form of the KS-11 form?
Answer: Everything Russian organizations, starting from 2013 has the right not to apply unified forms for documentary purposes business transactions, but to develop forms containing all required details, on one's own. Accordingly, if new form contains all the information required for disclosure; it can be used after the form has been previously approved by order of the head of the organization.

Why is the KS-11 act needed?

Form KS-11 is issued after the customer has checked and documented all measurement results (inspections, tests). What is accompanied by the act
As for the list of attached documents, the information regarding it is contradictory. Album unified forms(approved by Resolution of the State Statistics Committee dated November 11, 1999 No. 100), which contains both the KS-11 form itself and instructions for its use, refers to the list of documentation contained in the Temporary Regulations on the Acceptance of Completed Construction Facilities. However, in accordance with the letter of Rosgosstroy dated October 31, 2001 No. SK-5969/9, the previous letter of the department, which recommended this Temporary Regulation for use, was canceled. In this regard, the list necessary documents must be agreed upon by the parties when concluding the contract.
Based on the act, the contractor submits information to the statistical office that the construction of the facility is completed and all contractual obligations. In these reports, the asset put into operation is shown in the period when such a fact was recorded (meaning the fact of commissioning).
After this, the above fact is brought to the attention of municipal authorities executive power. Moreover, this is already done by the real estate user or the construction customer.

Not all buildings need to be issued KS-11 and 14. The list of objects is given in many laws. This includes the following buildings and systems:

When constructing such facilities, forms KS-11 and 14 are needed. Design documentation for such buildings can only be created and prepared by an individual entrepreneur, legal entities and companies that have special permissions to carry out and witness such work. Permitting documents are issued by the SRO authority.
Reception and delivery of completed work
Upon completion of construction, the developer provides the company with a work completion certificate - KS-11. You may also need form number 14. These are two different documents, their differences will be discussed later. The completion of construction is confirmed by the transfer of a package of documents from the contractor to the customer.
Under contract construction contract the contractor assumes the responsibility to construct the building or carry out repairs within the specified time frame. The customer takes on the burden of creating necessary conditions for this purpose, and then pays for the work. Also by Civil Code, as soon as the contractor reports the completion of the work, the company must accept it as quickly as possible, without delay. The results are accepted either at the very end of the work, or in stages - this is fixed in the contract.
The results of the work and its acceptance end with the signing of the acceptance certificate KS-11. There are also situations when one of the parties refuses to sign. So, when registering KS-14, the receiving party is always acceptance committee, and this does not exclude that in general there may be several parties to the document at once. In KS-11 there are often only two persons - the contractor and the customer, and they also put their signatures.
