  • a document confirming your gender change (birth certificate, repeated birth certificate) - if you are changing your passport due to a gender change;
  • application for the loss of a passport (it indicates where, when and under what circumstances the passport was lost or stolen, as well as the date and name of the authority to which the citizen contacted regarding the theft of the passport) - if you want to restore a lost or stolen passport.
