After you have contacted the consular office to find out the necessary documents, you need to fill out a visa application and register it. The easiest way is to do this using the official e-Konsulat website. On it you can fill out a form, which you will later print out and present at the consulate along with other necessary documents.

Some details to fill out:

  • Everything in the form is filled out in Latin letters;

Choose registration only if you are going to submit documents through the Polish embassy or consulate. If you are going to submit documents through a visa center, then choose to fill out the form.

When you select “Fill out the form,” the system displays this warning message. Don't be alarmed.

If you select “Register form”, a page will appear with instructions for filling out the form and protection from robots. As you know, the e-konsulat system is constantly subject to attacks by robots, so the Polish side is taking all possible measures to prevent these problems. When the defense has been passed and the system has approved that you are a person, a window will appear where the applicant will be asked to select the type of visa requested and the date for visiting the diplomatic mission of Poland.

When all the formalities are settled, it’s time to start filling out the application form. The first set of questions concerns personal data. It’s best to keep your passport handy so you don’t misspell your first or last name. For simplicity, we have marked in red all the items that must be filled in in capital letters.

Please note that here you must indicate the real purpose of the trip, according to which you will collect the rest of the package of documents. If you are applying for a “Purchases” visa, in the purpose of travel section, select “Other” and write ZAKUPY below.

Next, proceed to filling out the “Travel Information” section. Indicate everything as your trip suggests: dates of entry/exit - the item implies the validity period of the requested visa (in our example, a visa is requested for a year), if you are traveling by invitation, then write the same dates as in the invitation.

“EU citizen data” is filled in only if your trip to Poland is by invitation. If you are traveling on your own, then check the box next to “does not apply.”

The last thing that remains for you to do is to check the boxes as an agreement that you are familiar with all the rules and accept them.

Well, that’s all, the form is filled out, at the very bottom of the page there are three buttons: Correct – Write down – Cancel.

If you are sure that the specified data is correct, then click Write down. If you missed some important points that should not be skipped, the system will give you this warning message. Incorrectly filled fields will be highlighted in red. Just correct them and click the Record button again.

If everything is correct, a window will appear to save the profile. Save it and then print it. Remember that the system automatically assigns a name to the file. If you are filling out several forms for several applicants, then make sure that all your forms are saved under different names.

And one last thing. Do not under any circumstances change the data in the application form saved on your computer. All corrections must be made online so that the printed form is no different from the electronic form. Otherwise, your visa will simply be denied.
